Triple Combination Pill Reduces Recurrent Stroke Risk by 39% in Landmark TRIDENT Trial

Results from the TRIDENT randomized controlled trial, led by The George Institute for Global Health, were published in The New England Journal of Medicine and presented at the World Stroke Congress in Barcelona.
Breakthrough Results in High-Risk Patient Population
The international study involved 1,670 patients who had experienced intracerebral hemorrhage (搜索) - a type of stroke (搜索) characterized by bleeding in the brain - and had systolic blood pressure (搜索) of 130-160mmHg. Participants received GMRx2, containing telmisartan 20mg, amlodipine 2.5mg, and indapamide 1.25mg, or a placebo, alongside standard care.
During an average follow-up period of 3 years, stroke (搜索) occurred in 4.6% of patients receiving GMRx2 compared to 7.4% in the placebo group, representing a 39% lower risk of recurrent stroke. The results showed one stroke was prevented for every 35 patients treated with GMRx2.
Professor Craig Anderson, Principal Investigator and Senior Professorial Fellow at The George Institute, explained: "Lowering blood pressure (搜索) is the only proven method to prevent another stroke (搜索), yet achieving good blood pressure control is a real challenge. One big issue is that the number and doses of antihypertensive medications are not increased when needed and doctors and patients struggle with complex pill regimens."
Superior Blood Pressure Control and Cardiovascular Benefits
The GMRx2 group achieved better blood pressure (搜索) control, with mean systolic blood pressure levels 9mmHg lower than the placebo group. The treatment was titrated to deliver "aggressive" blood pressure reduction, with primary endpoint curves beginning to separate after eight months.
Beyond stroke (搜索) prevention, patients treated with GMRx2 experienced a 33% reduction in major cardiovascular events, including non-fatal stroke, non-fatal heart attack and cardiovascular death. The drug demonstrated a good safety profile, with serious adverse events affecting 23.8% of GMRx2 patients compared to 26.8% in the placebo group. Concerns of fatigue, dizziness and falls were infrequent and occurred similarly between treatment groups.
Addressing a Critical Global Health Challenge
Almost 17 million people worldwide have experienced intracerebral hemorrhage (搜索), with over 3 million new cases annually. Among survivors, approximately one quarter will later die from recurrent stroke (搜索) or cardiovascular disease (搜索). The condition disproportionately affects people in low- and middle-income countries, where blood pressure (搜索) control is often poor.
Acute intracerebral hemorrhage (搜索) accounts for at least 10% of the 20 million new strokes worldwide each year and represents one of the most dangerous stroke (搜索) types, occurring at almost twice the rate in low- and middle-income countries compared to high-income nations.
Professor Jeyaraj Pandian, President of the World Stroke (搜索) Organization, commented: "TRIDENT is a major advance in showing the enormous benefits of effective blood pressure (搜索) control after an intracerebral hemorrhage (搜索), and a simple and effective strategy in which this can be achieved, with relevance to patients all over the world."
Regulatory Approval and Market Launch
GMRx2, also marketed as Widaplik (搜索), received FDA approval in June as the first single-pill triple therapy for hypertension (搜索) and is scheduled for launch in the final quarter of 2024. The drug is developed by George Medicines (搜索), an independent spin-out company from The George Institute for Global Health, headquartered in London.
According to company CEO Mark Mallon, the TRIDENT result "represents an important step towards understanding how enhanced blood pressure (搜索) management has the potential to reduce the risk of recurrent stroke (搜索)." Only around one in five patients treated for hypertension (搜索) worldwide can manage it effectively with current medicines.
Professor Anderson noted the potential for broader impact: "These study results have the potential to mark a real shift in how we manage blood pressure (搜索) following a stroke (搜索). We hope GMRx2 is approved for this indication by regulatory authorities throughout the world, and if so that it is widely used as an effective approach with the potential to improve the outcome for patients affected by intracerebral hemorrhage (搜索), and also ischemic stroke, across the globe."
