Inclusion Criteria

1.Aged 18 - 60years

2.Involvement of two or more dysfunctional organs.

3.Availability of blood samples to determine fasting insulin and glucose within two hours after ICU admission.

Exclusion Criteria

1.Patients with pre-existing diabetes mellitus, chronic renal failure malignancy, cirrhosis and autoimmune disorders.

2.Patients on corticosteroid therapy

3.Patients with pancreatitis

4.Inability to give consent
