Consenting adult inpatients age > 18 years on treatment for at least 6 months period with 2 or more select chronic diseases such as
a)Cardiovascular diseases (hypertension, coronary artery disease, peripheral artery disease, stroke, chronic kidney disease, heart failure)
b)Musculoskeletal disorders (rheumatoid arthritis, osteoarthritis)
c)Chronic respiratory disorders (COPD, asthma)
d)Diabetes mellitus
a)Patients who have psychiatric disorders, cognitive impairment or visual disturbance
b)Patients who are unlikely to follow up at 6 months (Eg: living in another location)
c)Critically ill patients
d)Patients who have cancer, HIV or TB
e)Patients who donâ??t know languages such as English, Kannada, Tamil, Telugu, Hindi and Marathi
f)Patients or care givers without mobile phones or unable to read English messages
