Existent severe illness from the neurological or neurosurgical spectrum defined as early rehabilitation Barthel Index (ERBI) < 35 points, a cough deficiency and/or retention of secretion, identified by peak cough flow <270l/min. or diagnosed by application of the airway clearance score (ACS) >7 or cough >1 on ACS (Coplin et al. 2000, Windisch et al. 2017, Tanwar et al. 2019)
Age <18 years, known bullous pulmonary emphysema, presence of pneumothorax during current clinical treatment or instable sternum.
Amyotrophic lateral sclerosis as well as other progressive neuromuscular disorders and paraplegia, continous intracenous sedation, orally intubated patients, expected death on the ICU
