Eligibility Criteria
Inclusion Criteria
- •1\.Patient who has given written informed consent.
- •2\.Patients with diagnosed CKD (As per treating physician and/orAlbuminuria \- AER \>30 mg/24 h and/or Electrolyte and other abnormalities), attending nephrology OPD
Exclusion Criteria
- •1\. CKD patients admitted in ward
- •2\. Patients who are planned for renal transplant
