简要总结

The assessment of intravascular fluid volume status in patients admitted to ICU is essential,not only for diagnosis and treatment but also to improve patient’s monitoring. Although clinical examination and other static(CVP, pulmonary artery wedge pressure,stroke volume) as well as dynamic measures (stroke volume variation, pulse pressure variation) are used but USG is convenient, safe,non invasive,economical, easily obtainable, performed at bedside and encouraging results have been observed with USG based parameters used for CVP estimation. So we intend to do ultrasonographic measurements of IJV and subclavian vein to observe fluid volume status and respiropha variations in ICU patients.

入选标准

  • Critically ill patients in ICU Spontaneously as well as mechanically ventilated patients.

排除标准

  • 1 patients for whomever ultrasound examination would not be appropriate 2 obstetrics patients 3 patients undergone abdominal surgeries and ascitis 4 History of radiotherapy and surgery of neck region 5 Cardiac arrythmias 6 DVT in upper extremities 7 Clinically significant tricuspid and mitral regurgitation 8 patients having PaO2/FiO2 less than 200 9 patients with right sided CVP in situ.

时间窗: 5-10 days according to the length of stay in ICU

次要结局

  • Implications of respirophasic variations in dimensions of subclavian vein and IJV on fluid status assessment in spontaneously breathing and mechanically ventilated patients.(To observe the best predictor ( USG guided subclavian or IJV) in predicting fluid status in mechanically ventilated and spontaneously breathing patients in ICU.)
