Inclusion Criteria
1. patient over 18 years of age
2. patient receiving mechanical ventilation for at least 24 hours
Exclusion Criteria
1.patient with a tracheostomy tube, intubated with more than once
2.systolic bp more than 180 and less than 90 mmHg
3.decrease level of consciousness.
4.patient with neuromuscular disease
