The participants were recruited into the study based on the following inclusion criteria:
1. Being ACS pateints who was first diagnosed
2. Being over 20 years
3. Having no serious complication including heart failure, Cardiac arrhythmias, severe hypertention
4. Having Killip class 1 /Ejection fractio greater than 45%
5. Having no significant cognitive problems, i.e. dementia. screening by The Chula mental Test
6. Being contacted by telephone and line
7. Being able to read, write and communication in Thai language
The participants who developed serious cardiovascular condition, such as sever heart failure, cardiac arrhythmia and depression during the study period would be excluded from the study. Moreover, participants who do not participate in all seesions of intervention would be also excluded.

Name Time Method Readiness for discharge Day 3 after PCI (Discharge Day) and week 2 after discharge the Readiness for discharge among patients with acute coronary syndrome questionnaire which developed by researcher was used to assess Readiness for Hospital discharge of participact in this study.,Adaptation Week 2 and 4 after discharge the Adaptation for ACS patients questionaire which developed by researcher was used to assess adaptation after discharge of participact in this study.,Clinical symptoms week 2 and 4 after discharge the Clinical symptoms questionnaire which developed by researcher was used to assess clinical symptoms after discharge of participact in this study. It is a dichotomous question that asked for yes/no.
