Inclusion Criteria
1. Adult patients, diagnosed with HF within the last 12 months.
2. Ability to be accessed by mobile phone and internet signals.
3. Ability to use the mobile app either with or without assistance from a caregiver.
4. Having a Thai Mental State Examination (TMSE) score >23
Exclusion Criteria
1. Having cancer, chronic kidney disease, or surgery planned within two months, and/or were on dialysis.
2. Submitting self-monitoring data fewer than 30 days of the total 37 days.
