Inclusion Criteria
1.Tracheostomized Traumatic brain injury patients in ICU.
2.Patients with intact cough reflex.
3.Spontaneously breathing patients on T-piece or on room air.
Exclusion Criteria
1.Cervical spine injury.
2.HR below 60/min.
3.Arterial pressure less than 100/60 mmHg.
4.History of allergic reactions to study drugs.
5.Refusal to take part in the study
6.Patients with COPD /Asthma /Diabetes/ Hypertension.
7.Patients on Neuromuscular blockers or any sedative drugs.
8. Patient on ventilator.
