Sixty patients of severe traumatic brain injury (TBI), between 18-60 years of age, admitted to neurotrauma intensive care unit (ICU), who are undergoing mechanical ventilation and having stable ICP ( < 20 mm Hg) and haemodynamics (Systolic blood Pressure (SBP) >= 90 mm Hg) will be enrolled for this prospective, randomized double blind study
Exclusion criteria will include patients having any chest injury interfering with effective chest physiotherapy ie. fractured ribs or fracture dorsal spine, percutaneous emphysema, presence of haemothorax, pneumothorax, or a chest drainage tube. Patients with history of severe cardio-respiratory disorders and those having hypotension (SBP < 90 mm Hg) or hypoxemia (PaO2 < 60 mm Hg) or SpO < 90% will also be excluded
