- •\- CKD stages 1\-4 for more than 3 months
Exclusion Criteria
- •\- high health literacy (checklist/screener),
- •\- major cognitive problems,
- •\- too severely ill according the physician,
- •\- life\-threatening conditions (e.g. cancer)
