Eligibility Criteria

Inclusion Criteria

  • INCLUSION CRITERIA
  • Cluster Level:
  • 1\.Villages under Rural field practice area of AIIMS Bhuabaneswar
  • 2\. Persons with diagnosed hypertension aged between 30\-60 years
  • 3\. Residing in the same hamlet of the village for last six months.
  • 4\. Patients willing to give written informed consent to participate in the study.

Exclusion Criteria

  • EXCLUSION CRITERIA
  • Cluster Level:
  • 1\.Villages under Rural field practice area of AIIMS Bhuabaneswar with distance less than 5 kms from Community Health Centre.
  • Individual Level
  • 2\. Patients unable to communicate (b/o hearing loss, diagnosed psychosis etc.)
  • 3\. Patient with diagnosed cases such as myocardial infarction, stroke, heart failure, advanced liver disease.
  • 4\. Debilitated patients
