Eligibility Criteria
Inclusion Criteria
- •Cluster Level: 1\.Villages under Rural field practice area of AIIMS Bhubaneswar.
- •Individual level: 1\. Persons with diagnosed hypertension and/or diabetes mellitus and /or dyslipidemia aged between 30\-60 years. 2\. Residing in the same hamlet of the village for last six months. 3\. Patients willing to give written informed consent to participate in the study.
Exclusion Criteria
- •Cluster Level:
- •1\.Villages under Rural field practice area of AIIMS Bhubaneswar with distance less than 5 kms from Community Health Centre. Individual Level
- •Individual level:
- •1\. Patients unable to communicate (b/o hearing loss, diagnosed psychosis etc.)
- •2\. Patient with diagnosed cases such as myocardial infarction, stroke, heart failure, advanced liver disease.
- •3\. Debilitated patients
