Eligibility Criteria

Inclusion Criteria

  • Cluster Level: 1\.Villages under Rural field practice area of AIIMS Bhubaneswar.
  • Individual level: 1\. Persons with diagnosed hypertension and/or diabetes mellitus and /or dyslipidemia aged between 30\-60 years. 2\. Residing in the same hamlet of the village for last six months. 3\. Patients willing to give written informed consent to participate in the study.

Exclusion Criteria

  • Cluster Level:
  • 1\.Villages under Rural field practice area of AIIMS Bhubaneswar with distance less than 5 kms from Community Health Centre. Individual Level
  • Individual level:
  • 1\. Patients unable to communicate (b/o hearing loss, diagnosed psychosis etc.)
  • 2\. Patient with diagnosed cases such as myocardial infarction, stroke, heart failure, advanced liver disease.
  • 3\. Debilitated patients
