Eligibility Criteria

Inclusion Criteria

  • •Malades hospitalisés en réanimation dans l'un des servives participants à l'étude et remplissants les critères suivants:
  • •\- patient intubé sous ventilation mécanique depuis au moins 24 heures
  • •\- spO2\> \= 90% avec FiO2 \< \= 50% et peep\= 8 cm H2O
  • •\- hémodynamique stable sans vasopresseurs ( dopamine \<\= 10 gamma/kg/minute et la dobutamine \<\= 10 gamma/kg/minute sont toutefois tolérés) ni remplissage vasculaire ( défini comme la perfusion rapide d'au moins 500 ml de macromolécules ou 1000 ml de sérum physiologique) dans les 12 heures précédentes.
  • •\- sédation interrompue ou abaissée durant les 48 heures précédentes (l'analgésie peut toutefois être poursuivie)
  • •\- état neurologique stable
  • •\- température corporelle \> 36°C et \< 39°C
  • •\- consentement éclairé signé par le patient ou par un proche
  • •\- couverture par la sécurité sociale au régime équivalent
  • •Are the trial subjects under 18? no

Exclusion Criteria

  • •Critères de non\- oinclusion définitifs:
  • •\- grossesse ou allaitement
  • •\- âge \< 18 ans
  • •\- allergie connue au furosémide ou aux sulfamides
  • •\- trachéotomie à l'inclusion
  • •\- encéphalopathie hépatique
  • •\- oedème cérébral, hydrocéphalie aiguë
  • •\- myasthénie ou polyradiculonévrite aiguë ( syndrome de Guillain Barré)
  • •\- décision d'arrêt thérapeutique
  • •\- arrêt cardiaque prolongé avec pronostic neurologique réservé
