Inclusion Criteria

Age more than 18. Patients with symmetrically, bilaterally, horizontal or vertical completely impacted lower third molars with difficulty index ranged from 7 to 10 based on Pederson's description, who in need of surgical extraction (c) no pericoronitis or periapical lesions. Patients who are cooperative and able to attend the follow-up visits. No previous history of systemic diseases or under systemic drugs

Exclusion Criteria

(a) Age less than 18
(b) Patients with asymmetrically, bilaterally, horizontal or vertical completely impacted lower third molars.
(c)pericoronitis or periapical lesions.
(d) previous history of systemic diseases or under systemic drugs.
