Inclusion Criteria
Respiratory failure within 48hrs post extubation requiring non invasive respiratory support (i.e., High flow nasal oxygenation)
Exclusion Criteria
Age < 18yrs
Elective intubation for therapeutic or diagnostic purposes
Do not intubate orders
Accidental extubation
Presence of hypercapnia (PaCO2 > 45 mmHg)
Acute respiratory failure secondary to asthma
Chronic obstructive pulmonary disease (COPD) exacerbation
