Thai's patient age equally to or more than 18 years old.
Admit to intensive or intermediate care in Ramathibodi hospital
Intubated
Consent to the trials.
During adjustment of ventilator needs
On high ventilator setting (PEEP >12, FiO2 >0.6)
Needs nebulization more frequent than 30 minutes
Needs suction more frequent than 30 minutes
Respiratory rate more than 20 times/minutes
Pain with Numeric pain rating scale or visual analogue scale more than 6
Wish not to participate
