Inclusion Criteria
Eighteen documents of physician handover session before intervention and 18 after the intervention randomly selected for monitoring.
Exclusion Criteria
Clinical documents of handover session which physicians do not agree with the voluntary participation in the study.

Name Time Method The quality of information registered in the medical handover recording; using a tool with following criteria: patient identification, hospital location, clinical history, facts about the hospitalization, diagnosis, alerts (severe conditions, critical problems or values, adverse events, allergies), plan care.;Security of information recorded on the medical handover; using the following criteria: identification of document editors (authenticity and certification), access profile, and traceability.

Name Time Method enght of the transfer of care sessions between doctors shifts; evaluated by an external observer using a chronometer.
