Inclusion Criteria
Treatment in an EMANet emergency department during the study period, outpatient care received in the emergency department, triage categories 3, 4 or 5 (Manchester Triage System), no attachment to a primary care provider (self-reported, EMAPREPARE I), arrival at the emergency department by emergency medical services (EMAPREPARE II).
Exclusion Criteria
Limited or absent ability to give informed consent, proficieny in the German language inadequate for survey participation.

- Primary Outcome Measures
Name Time Method Acceptability and readiness to participate in the intervention (EMAPREPARE I), measurement time points t0: emergency department visit, t1: follow-up at 12 months.
- Secondary Outcome Measures
Name Time Method Medical, demographic and psychosocial characteristics of patients, reasons for utilization of the emergency department in cases of ambulatory care demands, primary care involvement, flexibility towards alternative care offers, health care utilization prior to the index visit (all: at t0), in EMAPREPARE I additionally: health care utilization reported by patients and primary care providers after the index visit (at t1).
