Patient with consent in the study
At least age18
The medical history is reviewed and the patient is in a healthy state ( asa I, II)
In dental history, including healthy patients with mandibular first or second molars with acute irreversible pulpitis and no PDL enlargement in periapical radiographs and without sensitivity to percusion
Lack of allergy to epinephrine and lidocaine along with epinephrine
Molars require treatment of the root that can be directly repaired with resin composite without requiring fiber post
Having opposite tooth
Systemic condition (ASA III,IV)
Age under 18
Tooth with containdication of resin composite
Periodontal problems , gingival recession , and the surrounding bone with deep pocket
Necrotic teeth with infection and dental inflammation
Allergy to the anestethic substance
Use any pain medication at 12 hours before referral to dentist
Chronic drug use of psychotropic substances and alcohol
Having a crown
Pregnancy and lactation
Pain in another part of the face or mouth
