1.Age more than 18 years
2.Either gender
3.Diagnosed with stage 4 lung cancer
4.Patients have agreed to participate in the study and have signed written informed consent.
5.Admitted or attending Palliative care OPD at IRCH
6.Patients already on Step 1 WHO analgesic ladder
7.Opioid-naïve patients
8.NRS >4 (Moderate to severe pain)
9.Somatic, visceral or mixed pain
1.Diagnosed mental illness or cognitive impairment
2.Patients refusal to give consent/assent to participate in the study
3.Patients with delirium or obtundation
4.Co-existing liver or renal diseases
5.Patients requiring intravenous(IV) drugs or IV morphine titration
6.Pure neuropathic pain
