1. Fulfilling ICD- 10 criteria for Alcohol/Opioid Dependence Syndrome
2. Education- who can at least read Hindi/English
3. Patients having access to functional mobile phone and self reported capacity to access phone calls and messages
4. Patients who have given consent during admission
5. Patients who are asked to follow up in NDDTC by treating team
1. Patients who are asked to follow up somewhere else and not at NDDTC
2. Patient who cannot give consent due to co-morbid Psychosis, or significant cognitive dysfunction
3. Patients transferred to another ward from NDDTC
