Eligibility Criteria

Inclusion Criteria

  • 1) AGE >18years
  • 2) Elective cardiac surgery
  • 3) Surgical intervention performed with CPB, aortic cross-clamping 4) Valvular surgery, coronary surgery, surgery on the ascending aorta 5) Median sternotomy and bi-pulmonary ventilation

Exclusion Criteria

  • 1) Patient refusal
  • 2) Non elective cardiac surgery
  • 3) Previous cardiac surgery
  • 4) Anticipated circulatory arrest.
  • 5) Patients with BMI >30
  • 6) Patients with end-stage chronic kidney disease (defined as need for dialysis) 7) Patients with known respiratory diseases (ongoing respiratory infections, asthma, COPD, OSAS)
  • 8) Patients already intubated before arrival in operating theatre
  • 9) Previous pulmonary resection
  • 10) Patients with a preoperative oxygen saturation <90%, or Pa O2 < 60 mmHg without supplemental oxygen, or a PaO2 /FiO 2 ratio <300, or PaCO 2 > 45 mmHg
  • 11) Patients with hepatic disease, defined as elevated liver enzymes (higher than two reference intervals)
  • 12) Patients with pulmonary hypertension (defined as preoperative estimated systolic pulmonary artery pressure >55 mmHg)
