1.Age ≥ 40 years
2.COPD patients, Diagnosis by spirometry recorded post bronchodilator FEV1/FVC < 70%
3.Respiratory failure with mechanical ventilation more than 24 hours,
4.Ability to tolerate SBT for 1 hour before ultrasound
5.Patient readiness for weaning must have met all criteria: using FIO2 < 0.5, and PEEP ≤ 5 cmH2O, PaO2/FIO2 > 200, respiratory rate ≤ 30 breaths/minute, stable hemodynamic (absence or low dose vasopressors required), good consciousness and cooperative, minimal secretion and effectiveness of cough
1.The patients or relatives of the patients refused to participate
2.Known case of Neuromuscular weakness
3.On tracheostomy tube
