i.Patients admitted with acidotic, hypercarbic COPD exacerbation.
ii. Age > 18 yrs and = 8yrs.
1.Pre-existing diaphragmatic dysfunction or neuromuscular diseases affecting diaphragmatic function
2. Pregnant women
3. Contraindications to the sniff test or absence of a window on ultrasound examination
4. Inability to sit up for the fluoroscopy or USG examination
5. Patients who are frail, bedbound, end stage organ damage, malignancy
6.NIVO score being either too low (0–2) or too high (7 to 9).
