From the approval date until December 31, 2025, patients aged 65 or older with kidney disease or internal medicine disease who are scheduled to be discharged to their homes after receiving rehabilitation prescriptions. Kidney disease refers to patients with chronic kidney disease admitted to the nephrology department, while internal medicine disease refers to patients admitted to the general internal medicine department. The use of assistive devices is not a factor, and the patient must be able to walk 10 meters or more independently or with proximal supervision. Patients must have no cognitive impairment, be able to fully understand the research plan and its contents, and be able to give consent themselves.
Individuals with a diagnosis of dementia.
Those with a history of neurological disorders that may affect physical function.
Those with comorbid orthopedic disorders (including foot lesions).
Patients with chronic kidney disease undergoing dialysis.
Individuals for whom consent could not be obtained before the start of the examination.
