Patients with an increased risk for or who suffer from (cardio)vascular disease
(i.e. cardiovascular disease, diabetes, hypertension, high cholesterol) or with
osteoarthritis with or without arthroplasty visiting the participating
outpatient clinics Internal Medicine or Orthopedics of Amsterdam UMC and UMC
Groningen are eligible to be recruited. In order to be eligible to participate
in this study, a subject must meet all of the following criteria:
- Aged 18 years and above
- who have a self-reported BMI >=25 kg/m2 at initial screening; or smoke;
- who consent to randomisation.
A potential subject who meets any of the following criteria will be excluded
from participation in this study:
- are not able to walk at least 100 meter safely (e.g. wheelchair bound);
- are pregnant;
- are cognitively unable to comply to a healthy lifestyle intervention referral
or to complete measures;
- does not speak the Dutch or English language.

Name Time Method <p>Primary outcome measures are changes at 12 months to the adapted-Fuster-BEWAT,<br /><br>which is a composite health risk and lifestyle score, which consists of six<br /><br>components: resting blood pressure, objectively measured physical activity,<br /><br>objectively measured sitting time, body mass index (BMI), fruit and vegetable<br /><br>consumption, and smoking. </p><br>
- Secondary Outcome Measures
Name Time Method <p>Secondary outcomes are cardiometabolic biomarkers, anthropometric measures,<br /><br>behavior measures, psychological measures and patient reported outcome measures<br /><br>(PROMs). Cost effectiveness will be assessed and a process evaluation will be<br /><br>conducted. </p><br>
