Inclusion Criteria

1. They are> 45 years old
2. They have an expectation of postoperative stay> at 24 hours for surgical reasons
3. Meet any of the following 6 criteria:
A. history of coronary artery disease
B. Vascular artery disease
C. History of Stroke
D. Hospitalization by ICC within 3 years prior to randomization.
E. Patients going to vascular surgery

Exclusion Criteria

1. Contraindication for metoprolol
2. There is a clinical need for beta-blocker use preoperatively or during the first 30 days postoperatively
3. Previous adverse reaction to a beta blocker
4. Coronary revascularization surgery in the previous 5 years and no evidence of cardiac ischemia since surgery
5. Patients who undergo low-risk surgical procedures
