Inclusion Criteria

The patient:
- stroke is the main cause of his dependency and has occurred longer than 3 months ago.
- lives in a nursing home in a 'somatic unit' (a unit for residents who have mainly physical handicaps);The informal caregiver:
- is the person to be contacted or representative of the patient.
- is closely in touch with the care for the patient.;The professional:
- is responsible for the daily care of the patient

Exclusion Criteria

The patient:
- has severe communication deficits
- can not be loaded due to severe depression or bad physical condition
- is not able to give informed consent

<p>For the quantitative study the primary outcomes are:<br /><br>a) the functioning of the nursing home resident who suffered a stroke:<br /><br>physical, cognitive, emotional, social and communicative functioning, as well<br /><br>the patient's capacity of decision making<br /><br>b) the current care and treatment<br /><br><br /><br>For the qualitative study the primary outcomes are:<br /><br>a) the needs for care from the perspective of the resident<br /><br>b) the needs for care from the perspective of the informal caregiver<br /><br>c) the needs for care from the perspective of the professional</p><br>
