Definitive diagnosis of heart failure by a physician
Diagnosis of heart failure in less than five years
New York Heart Association (NYHA) class II / III or American College of Cardiology stage B / C of heart failure
Ability to read and write Persian language
Access and ability to use smart phones
Absence of diagnosed cognitive disorders
Absence of irreversible vision and hearing disorders
Absence of comorbid disease that requires specialized palliative care such as cancer
Death or withdrawal of participants from the study
Lack of cooperation during the research (non-participation in two consecutive webinars or non-participation in Social Media Groups)
The patient enter the acute phase of the disease

- Primary Outcome Measures
Name Time Method Quality of life. Timepoint: Before the intervention, immediately after ending the intervention. Method of measurement: Persian version of the Kansas City Cardiomyopathy Questionnaire (PKCCQ) and Functional Assessment of Chronic Illness Therapy–Palliative-14 (FACIT-Pal-14).

Name Time Method Mood Status (Anxiety and Depression). Timepoint: Before the intervention, immediately after ending the intervention. Method of measurement: Hospital Anxiety and Depression Scale (HADS).;Number and duration of emergency hospital readmission of patients. Timepoint: immediately after ending the intervention. Method of measurement: Self-Report of Patients.
