Inclusion Criteria
1.All patient above 18 years requiring mechanical ventilation > 48 Hours
2.Anticipated ICU stay >96h
3.Patients with Acute illness; onset of symptoms <72 hours.
Exclusion Criteria
1.Readmission in to ICU
2.Pregnancy
3.Patients who stayed in the hospital =5 days in the past 2 weeks and not ambulating independently prior to illness that led to this ICU admission
4.Unable to obtain ultrasound images
5.Patient/ attenders who do not consent
