1. COPD patients stage moderate to severe with hypertension
2. Aged between 40-80 years
3. Good communication and cooperation
4. Good cognition with MMSE†Thai 2002 (Mini †Mental State Examination: Thai version) score more than 17 points with full scores are 30 points.
5. Stable COPD without acute exacerbation in the previous 4 weeks
6. Independent physical activity
1. Medical history related with contraindication of exercise including significant resting ECG suggesting ischemia, recent myocardial infarction, or cardiac event, unstable angina, uncontrolled dysrhythmias with symptoms or hemodynamic compromise and symptomatic heart failure, symptomatic severe aortic stenosis, acute myocarditis or pericarditis, and suspected or known dissecting aneurysm
2. Using oxygen therapy
3. History of renal disease or renal dysfunction, neuromuscular disease: cerebrovascular accident, spinal cord injuries
4. Medical history of pneumothorax, recent lung surgery (within 12 months), and spontaneous rib fractures
5. Pulmonary hypertension or pulmonary edema which unrelated with COPD, pulmonary infection, pneumonitis, pulmonary fibrosis with symptoms, active lung tuberculosis, and interstitial lung disease with symptom
6. Musculoskeletal disorders including; severe osteoporosis, limit shoulder flexion ≤ 90o, Hip or knee Osteoarthritis with pain on movement, Back pain with pain on movement, hand function impairment
7. Secondary hypertension
8. Visual impairment, hearing impairment, Balance impairment
9. Pulse oxygen saturation less than 90% at resting
10. Current smoking
11. Current another exercise training program which effect to results of this study
