Dual CD19/CD22 CAR-T Therapy Achieves Durable Remission in Refractory Plasmablastic Lymphoma Case

Treatment Journey and Disease Progression
The patient initially achieved short-term remission with first-line therapy consisting of bortezomib combined with etoposide, dexamethasone, cyclophosphamide, and doxorubicin. However, following disease progression, second-line treatment with daratumumab combined with GemOx showed no response.
After the second treatment failure, tumor biopsy analysis revealed strong positive CD22 (search) expression and partial positive CD19 (search) expression, providing the molecular rationale for dual-target CAR-T cell therapy. The patient subsequently received CD19 and CD22 CAR-T therapies as third-line treatment.
Clinical Outcomes and Tolerance
The dual CAR-T therapy (search) demonstrated remarkable efficacy, with the patient achieving durable complete remission that has persisted for more than one year. Importantly, the treatment was well-tolerated, with the patient showing good tolerance throughout the treatment period.
Addressing Unmet Medical Need
Plasmablastic lymphoma (search) represents a rare and highly aggressive form of non-Hodgkin lymphoma (search) associated with poor prognosis. Traditional chemotherapy has demonstrated limited efficacy for PBL, and there is currently no standard treatment for patients with refractory or relapsing disease.
While CAR-T therapy (search) has shown promising outcomes in leukemia, lymphoma, and myeloma, evidence of its application in PBL remains scarce. This case report contributes valuable clinical evidence supporting the potential effectiveness of dual-target CAR-T therapy in this challenging patient population.
Clinical Implications
The successful outcome in this refractory PBL case suggests that CD19 (search) and CD22 (search) CAR-T therapies may represent an effective treatment option for patients with refractory or relapsing plasmablastic lymphoma (search).
