Liposomal Amphotericin B Shows Comparable Efficacy to Triazoles in Invasive Aspergillosis Treatment

A new multicenter retrospective study challenges the traditional treatment hierarchy for invasive aspergillosis (search), demonstrating that liposomal amphotericin B (L-AmB) achieves comparable survival outcomes to mold-active triazoles (search) when used as primary therapy. The research, published in Open Forum Infectious Diseases, analyzed 401 patients across two Italian academic hospitals over a decade, providing crucial real-world evidence for clinicians managing this serious fungal infection.
Study Design and Patient Population
The investigation evaluated adult patients with proven or probable invasive aspergillosis (search) treated at two tertiary academic hospitals in northern Italy from January 2013 through December 2022. The patient cohort had a median age of 65 years, with 60.8% being male. Most cases were classified as probable invasive aspergillosis (96.0%), while proven disease accounted for 4.0%.
The study population reflected the evolving epidemiology of invasive aspergillosis (search). Hematologic malignancy (search) remained the most common predisposing condition, affecting 151 patients (37.7%), followed by viral-associated pulmonary aspergillosis (search) in 120 patients (29.9%). Chronic steroid treatment and chronic pulmonary disease (search) were each present in 64 patients (16%).
Treatment Patterns and Modifications
Initial antifungal therapy consisted of mold-active triazoles (search) in 296 patients (73.8%), while 105 patients (26.2%) received liposomal amphotericin B. Among triazole-treated patients, voriconazole was used in 272 patients (91.9%) and isavuconazole in 24 patients (8.1%). No patients received posaconazole as primary therapy.
Treatment modification patterns differed significantly between groups. In the liposomal amphotericin B group, 66 patients (62.9%) switched therapy at a median of 10 days after diagnosis, with transition to oral triazoles (search) being the most common reason (71.6% of those who changed therapy). In contrast, only 46 patients (15.5%) in the triazole group required treatment modification at a median of 13 days, most often due to adverse events (45.1%).
Survival Outcomes
Despite differences in treatment durability and switching patterns, survival outcomes were remarkably similar between groups. The overall 90-day survival rate was 58.8% for patients treated with triazoles (search) and 53.3% for those treated with liposomal amphotericin B. Using a landmark analysis beginning at day 7 post-diagnosis and inverse probability of treatment weighting to address confounding by indication, investigators found no statistically significant difference in mortality, with an adjusted hazard ratio for death of 1.43 (95% confidence interval, 0.87-2.33).
Clinical Implications
"Our findings indicate that L-AmB can be as effective and safe as triazoles (search) for the initial treatment of patients with invasive aspergillosis (search)," said Dr. Maddalena Giannella, full professor of infectious diseases at University of Bologna (search) and senior infectious diseases consultant at IRCCS Azienda Ospedaliero-Universitaria di Bologna.
The research addresses growing concerns about triazole resistance and drug interactions. "Considering the rise in drug resistance in invasive aspergillosis (search) and the issues related to the use of triazoles (search), particularly voriconazole, such as the need of therapeutic drug monitoring and drug-drug interactions, we deem these data reassuring that L-AmB is an effective alternative to initial triazole-based treatment," explained Dr. Matteo Rinaldi, clinical researcher at the University of Bologna (search).
Strategic Treatment Considerations
The study found that liposomal amphotericin B therapy was preferred for higher-risk patients, including those with hematologic cancers and those who had taken triazoles (search) prophylactically but still acquired invasive aspergillosis (search). Dr. Russell Lewis, associate professor of infectious diseases at University of Padua, noted that "toxicities with L-AmB are avoided with switching to triazoles once the pathogen and/or the susceptibility is known."
The findings have particular relevance given the expanding patient population at risk for invasive aspergillosis (search). As the authors noted, "described risk factors for IA have broadened beyond classical disease states such as hematologic malignancies and solid organ transplantation to include patients with chronic pulmonary diseases, chronic steroid treatment, and prolonged intensive care unit hospitalization."
The research reinforces the importance of antifungal stewardship and early reassessment of therapy, given the high rates of treatment switching observed. While triazoles (search) remain the standard first-line therapy according to current guidelines, this study provides evidence that liposomal amphotericin B represents a viable alternative for selected patients, particularly in the context of rising azole resistance and complex drug interactions.
