Olorofim Shows Promise for Severe Valley Fever in Salvage Trial, Offering Potential Curative Approach

As Valley fever (search) cases surge across California and the western United States, researchers report that an experimental antifungal drug called Olorofim has shown significant promise in treating patients with the most severe, disseminated form of the disease—offering a potential paradigm shift from lifelong suppression to fungal elimination.
The findings, published this week in the Annals of Internal Medicine, come from a multi-state salvage trial led by researchers including Dr. Fariba Donovan, associate professor at the University of Arizona College of Medicine (search) and a researcher with its Valley Fever (search) Center for Excellence.
A Different Mechanism of Action
Current standard-of-care antifungal medications for severe Valley fever (search), such as fluconazole and itraconazole, work by suppressing fungal growth. Patients typically must take these drugs for the rest of their lives, and the medications themselves carry significant side effects, including hair loss and cracking, painful, dry skin.
Olorofim operates through a fundamentally different mechanism. "Olorofim actually kills the fungus," Donovan said. "Olorofim is blocking the RNA and DNA synthesis, and by doing so, blocks the protein synthesis, and technically can eliminate the fungus."
The drug was originally developed for immunocompromised patients vulnerable to fungal infections. A prior phase III study in immunosuppressed patients infected with Aspergillus demonstrated that Olorofim worked as well as standard treatments but with significantly fewer side effects.
Trial Design and Patient Population
The study enrolled 41 patients between May 2019 and August 2022. Strikingly, nearly all participants—95%—had been healthy before being exposed to Coccidioides (search), the soil-dwelling fungus that causes Valley fever (search). Only two had suppressed immune systems prior to contracting the disease.
"That's really important," Donovan emphasized. "It's a disease that can make previously healthy people really sick."
The severity of disease in the cohort was notable: the infection had spread to the brain or spinal cord in 30 patients, a complication known as disseminated disease that is extremely dangerous and difficult to treat. Thirteen patients were so critically ill that they required a ventriculoperitoneal shunt—a surgically implanted device used to drain excess brain fluid caused by the infection or to deliver medication directly into the brain fluid.
Clinical Outcomes
The results were encouraging. By day 42 of treatment, more than three-quarters of patients had either improved or stabilized. By day 84, 73% continued to demonstrate clinical success.
Dr. George Thompson, professor of medicine at the UC Davis School of Medicine (search), characterized the study as a "salvage" trial for patients who had failed current standard forms of treatment. "This was great, because we're always going to have patients who fail" the standard form of treatment, Thompson said. He expressed hope that "in the future, a head-to-head first-line trial can be done" to test Olorofim against other antifungals, mirroring the approach taken in the Aspergillus trial.
The Possibility of Finite Treatment
One of the most tantalizing prospects raised by the study is whether Olorofim could eventually be administered for a finite period rather than indefinitely. Thompson said it remains unclear whether patients treated with Olorofim will need to take it for life, but he is hopeful the drug might offer an option "to give people a final round of treatment so they can be off the drug."
Donovan echoed this cautious optimism: "We think that actually by using this medication, that has very minimal side effects, potentially at one point, although we are not there yet—we don't have enough data to say that potentially we can give the patient medication for a period of time and then stop it and patient should be okay."
A Growing Public Health Threat
Valley fever (search) is caused by inhaling spores of the Coccidioides (search) fungus, which thrives in dry soil across the western United States. While most exposed individuals never develop symptoms, roughly 40% do. The majority experience mild respiratory illness often mistaken for influenza or COVID-19, but approximately 10% develop serious lung problems, and 3% to 5% of those patients progress to disseminated disease affecting the brain and spine.
Because early symptoms mimic common viral illnesses, Valley fever (search) is frequently misdiagnosed, sometimes going unidentified and untreated for months. The risk is particularly acute for visitors to endemic regions—such as the San Joaquin Valley or Phoenix—who then return home to communities where the disease is uncommon.
"We encourage patients to ask their providers about Valley fever (search), and bring it up," Donovan advised, particularly if they are suffering from a lingering cold or experiencing severe migraine-like headaches that do not resolve.
Thompson noted he is observing an increase in both patient numbers and severe presentations, which his research suggests is likely attributable to the overall rise in case counts. He also pointed to a growing immunosuppressed population that is particularly vulnerable to fungal diseases.
Regulatory Status and Next Steps
Olorofim has not yet received FDA approval for the treatment of Valley fever (search). However, patients, doctors, and others may contact the drug company to request compassionate use access. Donovan indicated that more research is underway, with the goal of eventually securing FDA approval. Thompson expressed hope that additional Olorofim-type drugs will reach the market, given the increasing susceptibility of immunocompromised patients to fungal diseases.
