Study Reveals Significant Treatment Disparities in High-Risk Myelodysplastic Syndrome Patients

A comprehensive analysis of Medicare data reveals alarming disparities in the treatment of high-risk myelodysplastic syndrome (search) (MDS (search)), with only 16% of eligible patients receiving guideline-recommended hypomethylating agents (search) (HMAs (search)), according to research published in Blood Neoplasia.
The retrospective cohort study examined 7,935 patients with MDS (search) who received HMAs (search) between 2011 and 2014, uncovering significant gaps between clinical guidelines and real-world practice. Current guidelines recommend that all patients with high-risk MDS, who represent 30% to 40% of newly diagnosed cases, should receive HMA therapy.
Demographic Disparities Drive Treatment Access
The study identified pronounced demographic disparities in treatment access. Patients most likely to receive HMAs (search) were younger (ages 65-74), white, and male. Conversely, patients over 74 years old faced significantly lower odds of receiving treatment (adjusted OR 0.81 for ages 75-84; adjusted OR 0.41 for ages 85+) compared to those aged 65-74.
Gender disparities were equally striking, with female patients significantly less likely to receive HMAs (search) versus male populations (adjusted OR 0.81; 95% CI, 0.77-0.86). Racial disparities further compounded access issues, as Black patients (adjusted OR 0.70; 95% CI, 0.62-0.80) and patients of other races or ethnicities (adjusted OR 0.78; 95% CI, 0.68-0.90) experienced lower probabilities of receiving HMAs compared to white patients.
Clinical Factors Influence Treatment Decisions
Several clinical factors correlated with higher likelihood of receiving HMAs (search), including having 2 or 3 cytopenias (adjusted OR 2.08; 95% CI, 1.96-2.20) and undergoing bone marrow biopsy (adjusted OR 12.61; 95% CI, 10.78-14.74). However, frailty significantly reduced treatment probability, with moderate or severe frailty associated with an adjusted OR of 0.19 (95% CI, 0.13-0.20).
Nursing home residence also corresponded with lower likelihood of receiving HMAs (search) (adjusted OR 0.55; 95% CI, 0.47-0.63), highlighting additional barriers to optimal care delivery.
Treatment Adherence Falls Short of Guidelines
Among patients who did receive HMA therapy, adherence to recommended treatment duration proved problematic. Guidelines recommend completing at least 4 to 6 treatment cycles for maximum therapeutic benefit, with each cycle lasting approximately 28 days. For azacitidine, patients should receive the drug for 7 consecutive days per cycle, while decitabine requires 5 consecutive days.
The study revealed widespread non-adherence to these guidelines. Fifty percent of patients discontinued treatment by cycle 6, and an equal proportion failed to receive complete cycles. Complete cycles were defined as receiving at least 4 of 7 daily doses for azacitidine and at least 3 of 5 daily doses for decitabine.
Patients receiving decitabine were less likely to complete a minimum of 4 treatment cycles compared to those receiving azacitidine (adjusted OR 0.70; 95% CI, 0.62-0.78). Among azacitidine patients, 34% had discontinued treatment by the end of cycle 4, with approximately 50% discontinuing therapy by the end of 6 cycles.
Mortality Patterns Reflect Treatment Challenges
The mortality data underscored the severity of the disease and treatment challenges. In the azacitidine cohort, mortality rates reached 10% by the second cycle and 23% by the sixth cycle, climbing to approximately 30% to 37% in each cycle beyond cycle 9. By the end of 24 treatment periods, only about 5% of patients remained on treatment with either azacitidine or decitabine.
Study Population Characteristics
Among patients who received HMAs (search), 73% underwent treatment with azacitidine, 91.3% were white, and 71.5% had 2 or 3 cytopenias. Additionally, 71% had transfusion dependency, 50% resided in regions with medium-high to high poverty levels, and 80.4% lived in counties with populations exceeding 250,000.
Lead study author Sudipto Mukherjee, MD, from the Leukemia Program at Cleveland Clinic's Taussig Cancer Institute (search), emphasized the study's implications: "Our analyses revealed age-, sex- and race-related disparities in the receipt of HMA even after adjusting for MDS (search)-related clinical characteristics that are markers of disease severity, frailty, and known socioeconomic determinants of disparity."
The findings align with previous investigations that reported treatment rates between 14% and 18%, confirming persistent patterns of undertreatment in high-risk MDS (search) populations. The research highlights the urgent need to address systemic barriers preventing optimal care delivery and improve treatment adherence to enhance patient outcomes.
