Effects of Integrated Gaze Stabilization and Balance Training on Balance, Gait, and Falls Efficacy in Patients with Chronic Stroke: A Randomized Controlled Trial

A total of 39 patients with chronic stroke participated in this study and were randomly assigned to the integrated gaze stabilization and balance training group, the gaze stabilization exercise group, or the balance training group, with 13 participants in each group. The results showed that the integrated intervention group demonstrated greater improvements in static balance, dynamic balance, and gait ability than the single-intervention groups. In static balance, center of pressure area and center of pressure mean velocity decreased after the intervention. In dynamic balance, Berg Balance Scale scores increased and Timed Up and Go test times decreased. In gait ability, the integrated intervention group showed increased gait velocity and step length and decreased step time. However, step time asymmetry did not show significant changes over time or significant between-group differences. For falls-related psychological variables, after adjusting for baseline values, the integrated intervention group showed reduced concern about falling and improved balance confidence. Improvements in major balance and gait variables were also observed at the two-week follow-up. These findings suggest that integrated gaze stabilization and balance training may be an effective rehabilitation intervention for improving postural stability, functional balance, and gait performance in patients with chronic stroke.

- •Patients with chronic stroke who were more than 6 months after stroke onset
- •Participants who were able to walk independently or with a walking aid if needed
- •Participants with a Mini-Mental State Examination (MMSE) score of 24 or higher who were able to understand and follow the researcher’s instructions
- •Participants with a Berg Balance Scale (BBS) score between 25 and 45
- •Participants who understood the purpose and procedures of the study and voluntarily provided written informed consent
排除标准
- •Participants who had participated in another rehabilitation program or clinical study within the previous 3 months
- •Participants who had difficulty performing exercise due to severe musculoskeletal or cardiovascular disease
- •Participants who were unable to perform gaze stabilization exercises due to severe visual or hearing impairment
- •Participants who were considered inappropriate for study participation due to severe fatigue, pain, or communication difficulties
- •Participants who did not provide informed consent or were judged by the researcher to be inappropriate for participation
