First ever Stroke (ICD: I61, I63, I64)
NIHSS acute >0 Points
Frührehe-Barthel-Index <30 points
Stroke onset 72 hours
Subarachnoid hemorrhage (SAB), traumatic brain injury, hemorrhage into preformed cavities, other brain pathology significantly affecting prognosis (e.g., tumor, metastases).
Other disease determining the prognosis (e.g., acute myocardial infarction, endocarditis, cancer, multiple sclerosis)
Barthel index 30 points or above (thus not suitable for phase B)
Planned surgical procedures in the next 3 months
Cognitive impairment that makes informed consent impossible
Death as a result of the disease up to T1
Lack of patient compliance
Lack of informed consent from patient or relatives

- Primary Outcome Measures
Name Time Method The primary endpoint of the study is the motor function recovery after stroke, within the first 6 months of rehabilitation. For this purpose, a physical examination will be performed at three time points (T0, T1, T2). This includes, the Fugl-Meyer Assessment and the following scales: Barthel-Index, NIHSS, mRS, FIM and FAM. <br>T0 represents the time of study inclusion, here the patient is examined after consent on the stroke unit within the first 48 hours after stroke. <br>T1 defines the time at the end of the early rehabilitation in the Department of Neurology, University Hospital Cologne. <br>T2: represents the follow-up date after 6 months in the Polyclinic for Neurology, University Hospital Cologne
