Primary Outcome Measures

Primary endpoints are long-term care dependency according to the German law (SGB XI) and mortality. The incidence of long-term care is defined as the granting of care by the long-term care insurance of the AOK for persons, who were not initially recipients of care services. After completion of the project, these data are retrieved in an anonymous form from the performance statistics of the care fund for the intervention and control groups and are compared with each other after adjustment for the age and sex of the insured persons.<br>Mortality as the second primary endpoint is also determined on the basis of the administrative data of the health insurance Company at the end of the project and compared after adjustment for age and gender between the two groups.
Secondary Outcome Measures

The secondary endpoint is the inpatient treatment due to cerebrovascular diseases. Information on hospital treatment due to transient ischemic attacks, cerebral bleeding and ischemic stroke is collected in an anonymous form at the end of the project from the claims data of the health insurance company. The differences between the intervention and control group in the first occurrence of cerebrovascular diseases, the number of treatment episodes and the costs of inpatient treatment are compared after adjustment for age and sex.
