Inclusion Criteria

* Symptomatic SVD defined as a diagnosis of CADASIL
* Age 18 years or older
* Capacity to give written informed consent
* Independent in activities of daily living (Modified Rankin score * 3)

Exclusion Criteria

* Inclusion criteria are not met
* Unwillingness or inability to give written consent
* Pregnant women, women of childbearing age not taking contraception
* Contraindications to MRI
* Other major neurological or psychiatric conditions affecting the brain and interfering with the study design
* In case of clinical lacunar stroke syndrome other causes of stroke dan small vessel disease

<p>Measures of microvascular function at 7T MRI:<br /><br>- Difference in mean velocity and pulsatility index of perforator flow between<br /><br>patients and controls.<br /><br>- Difference in time course of the BOLD-fMRI signal to a visual stimulus in<br /><br>different cortical layers between patients and controls.<br /><br>- Difference in Cerebrovascular reactivity (CVR) to a hypercapnic stimulus<br /><br>between patients and controls.</p><br>
