Patients who are age at least 45 year old with either high atherosclerotic risk (group 1) or established atherosclerotic cv disease (group 2)
Group 1: Patients with The CORE Thailand is conducting to study the risk factors of atherosclerotic risk patients and patients with established atherosclerotic disease (coronary artery disease, cerebrovascular disease and peripheral arterial disease), the cardiovascular event rate in these population, the treatment and investigation for atherosclerotic disease greater than or equal to 3 risk factors as following:
1.1 Diabetes (type 1 or 2) or impaired fasting glucose
1.2 Hypertension (BP less than140/90 mmHg or currently treated with antihypertensive agent)
1.3 Chronic kidney disease stage I-IV
1.4 Dyslipidemia
1.5 Smoking
1.6 Male greater than or equal to 55 year old and woman greater than or equal to 65 year old
1.7 family history of premature atherosclerosis
Group 2. Patients with established atherosclerotic disease
2.1 Patients with cerebrovascular disease: transient ischemic attack, ischemic stroke
2.2 Patients with coronary artery disease: stable angina with evidence of myocardial ischemia, acute coronary syndrome, underwent coronary angioplasty/stenting or coronary artery bypass graft, coronary stenosis more than 50 percent by coronary angiogram or coronary CT angiogram
2.3 patients with peripheral arterial disease: intermittent claudication with Ankle Brachial Index less than 0.9, intermittent claudication with history of intervention (angioplasty, Vascular bypass graft or amputation)
1. Patients with recent episodes of acute cariovascular event (within 3 months)
2. Patients who are currently in blinded study
3. Patients with life expectancy less than 3 years from other illness such as AIDS or cancer
4. Patients with surgically indicated aortic aneurysm
5. Patients with difficulty of follow up
6.Patients who do not give consent
