Inclusion Criteria

All adult pts (age above 18yrs) who will receive invasive mechanical ventilation through endotracheal tube for more than 48hrs for hypoxemic respiratory failure

Exclusion Criteria

•Age less than 18yrs

•Type 2 respiratory failure

•Tracheostomized patients

•Profound neurological deficit and altered sensorium

•Contraindication to HFNC application (base of skull fracture, known nasal obstruction and nasal trauma)

•Psychiatric, agitated or non-cooperative patients
