First, an adult diagnosed with heart failure by a cardiologist.
Second, class I, II, III, and IV of the New York Heart Association Function Class in the United States.
Third, who has no history of stroke or brain disease surgery within the last month.
Fourth, who can communicate in Korean and use the phone after being discharged from the hospital.
Fifth, who can understand and fill out the questionnaire used in this study (in the case of the elderly, a person who can understand the content and answer verbally).
First, who with a life expectancy of less than 6 months
Second, who have other advanced diseases such as stroke and terminal cancer
Third, who with a psychiatric history, neurological disorder, and cognitive impairment that can affect understanding and responding to the questionnaire used in this study.
